Promotes a recovery based approach to member care that emphasizes hope, respect, self-direction and empowerment by providing peer support, education on community resources and other supportive services, to aid in members recovery from mental illness and/or addiction disorders. Guidance and direction is provided by the Regional Program Manager and Senior Community Programs Director. Essential Duties. Oversee all aspects of care to assigned members to ensure needs are identified and addressed to best support positive outcomes. Relate own recovery process in a positive and supportive manner through coaching, role modeling and mentoring in order to assist members in developing skills to achieve their goals. Work with recovery team members to assess mental and physical health and psychosocial needs and work collaboratively with the member/family, health home, community resources and other natural supports to develop an Individualized Service Plan (ISP) to address needs.
