This full-time position plays a key role in organizing and coordinating medical services and follow-up care for CHCY patients across all locations. Works with community providers to ensure that referred diagnostic testing is scheduled and completed timely.Provides patient education regarding CHCY services and specific program eligibility.Monitors progress in achieving goals to demonstrate improvement in the care and clinical outcome of the patients receiving care coordination.Assists with coordinating local marketing initiatives including advertising, materials development, and distributions such as flyers and patient handouts.
