This full-time position plays a key role in organizing and coordinating medical services and follow-up care for CHCY patients across all locations. Working closely with both practitioners and patients, the Care Coordinator ensures seamless communication, timely follow-up, and continuity of care. Works with community providers to ensure that referred diagnostic testing is scheduled and completed timely.Provides patient education regarding CHCY services and specific program eligibility.Monitors progress in achieving goals to demonstrate improvement in the care and clinical outcome of the patients receiving care coordination.Assists with coordinating local marketing initiatives including advertising, materials development, and distributions such as flyers and patient handouts. Helps plan and implement community events such as health fairs, group presentations, and professional education activities. Performs other job-related duties as assigned. Minimum QualificationsEducation:High school diploma or equivalent.Experience: No experience required.
